Practice Exercise: Writing a Crisis and Safety Plan

Practice writing some of the different elements of a Crisis and Safety Plan by completing the following fields. The content of the plan will be sent electronically to your supervisor for review. After the supervisor has reviewed your work and approved it, he or she will provide you with a code to enter on the following page, which will allow you to complete the module. This is required for all behavioral health participants completing this module as part of credentialing to become a CFT facilitator.

For this exercise, imagine a family you have worked with in the past (could be your own, one from a current or former job, or one that is made up). If you will be using a real family that you have worked with in a professional setting, please change names and identifiable information in order to protect confidentiality. Complete the following sections by writing what you think the Child and Family Team would have most likely planned if they had been completing a Crisis and Safety Plan with this family.

Predict: List the most likely thing to go wrong that would result in a crisis situation:
 

Functional Assessment: Describe functional details regarding what leads to the crisis behavior (antecedents, details, patterns relating to the crisis behavior).
 

Prevent: What will be done to help keep the crisis from occurring? (Include use of strengths, needs, culture, and informal supports).
 

Plan: What will be done (who, what, when, where, etc.)  if the crisis occurs despite the preventative efforts?
